Vasectomy in Tampa (Hillsborough County), Florida:

Hillsborough County Health Department

The Hillsborough County Health Department has an active Vasectomy Program with Title X financial assistance for men who qualify based on income and family size. Vasectomies are performed by Drs. Doug Stein and Alex Galante in their office at 288 Crystal Grove Boulevard in Lutz.

Until 2012, patients would report to one of the six health department clinics below for financial screening and vasectomy counseling. But the Health Department has consolidated some of its services. More recently, all eligibility evaluations and counseling are done at the Kelton Administrative Center, 4704-B Montgomery Avenue, (19 blks so. of Gandy, 3 blks e. of Westshore), Tampa, FL 33616 (map below).

1. Please call 813-559-4178 for an appointment.
Ms. Carry Simons is handling the eligibility screening and counseling. If you leave a message and do not get a return call within one business day, please call Clarence Gyden's direct line: 813-559-4167. If you leave a message for Clarence and do not get a return call within one business day, please call Dr. Stein's office at 813-536-1430.

2. At the time of your first call, Ms Simons will aks if you have had a physical exam and labwork within the past year. If not, she will ask that you have a physical exam and labwork at one of seven Suncoast Community Health Center clinics: (1) Palm River Road in Tampa, (2) Plant City, (3) Dover, (4) Ruskin, (5) Brandon, and new locations in Wimauma and Thonotosassa. The fees for the physical and for the vasectomy are based on income, so bring a tax return, paycheck stub, or other proof of income. (Most patients pay nothing.) Alternatively, you may have a physical and labwork at a walk-in-clinic at your expense. Either way, bring the results to Ms. Simons when you see her for counseling.

At least 30 days after signing your consent for the procedure, you may have your vasectomy in the Lutz office if Drs. Stein and Galante. Please call 813-536-1430 to schedule.
