No-Needle No-Scalpel Vasectomy

 Dr. Mike Ahmed of Palm Beach Family Physicians is kind enough to allow Dr. Doug Stein to use his office periodically to provide vasectomy services. Friday afternoon vasectomy dates are listed in our Outfield Schedule Page. The fee is $490. Some insurance plans (those with which Dr. Stein is a contracted provider) are accepted. The counseling process can be done online by (1) reviewing Dr. Stein's vasectomy page, (2) viewing the online counseling video, and (3) reading the vasectomy info sheet (this is a PDF file viewable with Acrobat Reader®, available here as a free download if you don't already have a copy of Acrobat Reader® on your computer). So only one visit is required, but you will have Dr. Stein's office, home, and cell phone numbers so that you can always reach him if you have questions after your procedure. You will also be given a mailer, so that you can send a semen sample to Dr. Stein's Tampa office 8 weeks after your vasectomy to be sure that your semen is sperm-free.

Directions:
From the north or south via Interstate 95 use Exit 66 and travel 0.8 miles west on Forest Hill Boulevard. Lakeshore Office Park will be on the right side behind Advance Auto Parts.

From the north via the Florida Turnpike use Exit 99 and travel east on Okeechobee Boulevard 1.7 miles to Military Trail. Go south on Military Trail 3.8 miles to Forest Hill Boulevard, then east on that 1.8 miles to Lakeshore Plaza, which will be on the left side, just before Advance Auto Parts.

From the south via the Florida Turnpike use Exit 93 and travel east on Lake Worth Road 3.6 miles to Military Trail. Turn left and go north on Military Trail 2.2 miles to Forest Hill Boulevard. Turn right and travel east on Forest Hill 1.8 miles to Lakeshore Plaza, which will be on the left side, just before Advance Auto Parts.

Lakeshore Plaza is just west of Lakeshore Office Park. When approaching from the west (from the Florida Turnpike), enter the complex at the sign for Lakeshore Plaza, just before Advance Auto Parts. Forest Hill Boulevard is divided at the sign for Lakeshore Office Park and you will not be able to turn left there if you do not enter at Lakeshore Plaza.

When approaching from the east, one comes upon the sign for Lakeshore Office Park rather suddenly while rounding a right bend in Forest Hill Boulevard. Turn in between the Lakeshore Office Park and Advance Auto Parts signs and drive back to the building.
Within the gated open-air corridor, Suite 100 is the first office on the right.

During other hours, or if you get a recording because someone else is on the line, you may leave a message and his staff will return your call. When you arrive at the West Palm Beach location, you will be asked to provide demographic and health history information as you are at any doctor's office. You may now register online before your visit. The benefits are numerous and listed in our On-line Registration Page.
Fees
The fee is $490. All patients receive a properly coded receipt that they can submit to their insurance carriers, where applicable, for reimbursement. A deposit of $100 is required to be scheduled. The deposit is fully refundable until 7 calendar days prior to the appointed vasectomy date, non-refundable thereafter for either cancellation or postponement. The deposit can be paid by providing a VISA/MasterCard number to Dr. Stein's Tampa office (813-972-1365 or 866-827-8463) or by mailing a check or money order payable to Douglas G. Stein, MD to his Tampa office (Douglas G. Stein, MD, PA; 3000 E Fletcher Avenue; Suite 330; Tampa, FL 33613). The balance of the fee ($390) is due on the day of the vasectomy, payable by cash or check. Credit cards are not accepted at this site, but you may prepay with Visa or MasterCard by calling your card number to the Tampa office (813-972-1365) during office hours: 9 AM - 6 PM Monday - Friday.  Friday vasectomy dates are listed in our Outfield Schedule Page. Some insurance plans (those with which Dr. Stein is a contracted provider) are accepted, but call your carrier and ask the questions here.
Financial Aid
Medicaid patients are accepted, as are Medicaid HMO patients with proper referral, but 2 visits are required to allow for the mandatory 30-day interval (required by Medicaid) between signing your consent and having your vasectomy. Most health insurance plans cover vasectomy. If you have insurance and you would like to know if vasectomy is covered under your plan, please follow the instructions here. If you have no health insurance and your income is low, you may qualify for financial aid under Title 10 from the Palm Beach County Health Department. Click for details. Dr. Stein is not a provider for the PBCHD, but you will be referred to another urologist.
