Complementary Contraception is a non-profit organization that supports voluntary contraception. The organization is kind enough to allow VasWeb doctors, specifically Mary Samplaski, to use the facility periodically (on certain Fridays) to provide vasectomy services. For exact dates, see our Outfield Schedule. The fee is $690, less for men with most health insurance plans, even if they have not met their deductibles. A deposit of $150 is required to be scheduled (see below). The counseling process can be done online by (1) reviewing our vasectomy page, (2) viewing the online counseling video, and (3) reading the vasectomy info & consent that you will sign when you arrive. So only one visit is required, but you will have the office, home, and cell phone numbers of all 3 doctors so that you can always reach them if you have questions after your procedure. You will also be given a mailer, so that you can send a semen sample to our Tampa/Lutz office 12 weeks after your vasectomy to be sure that your semen is sperm-free.

Please follow the steps on our Home Page, fill out our On-line Registration, then call our office at 813-536-1430. Staff is available to take your call between 9 AM and 4 PM, M-F. During other hours, our recording provides the cell phone numbers of our doctors.

Fees

The fee is $690, or, if you have health insurance through a company with whom Dr. Samplaski is a contracted provider (including Blue Cross, Aetna, and United HealthCare), the fee is the contracted rate for that company. Please click Payment Options for those rates. A deposit of $150, payable during the online registration, is required to be scheduled, unless your vasectomy is covered under Title 10 through a county health department, in which case a deposit is not required. The deposit is fully refundable until 7 calendar days prior to the appointed vasectomy date, non-refundable thereafter for either cancellation or postponement. The deposit is paid by providing a VISA/MasterCard number while registering online or by calling the number to our Lutz/Tampa office (813-536-1430). (If you do not have a VISA or MasterCard account, you may purchase a prepaid card at most discount stores.) The balance of the fee ($690, or your insurance company allowable rate, less $150) is due on the day of the vasectomy, payable by cash, check, or VISA/MasterCard. Financial Aid

If you have no health insurance and your income is low, you may qualify for financial aid under Title 10 from the Flagler County Health Department, from the Volusia County Health Department, or from the Brevard County Health Department, from the Seminole County Health Department.
