The Polk County Health Department has an excellent Vasectomy Program and may (depending on fund availability* each fiscal year) have Title X (Ten) financial assistance for men who qualify based on income and family size. Vasectomies are performed for Polk County Health Department patients by Drs. Doug Stein and Alex Galante at their home office in Lutz. For exact dates, see their Schedule Page. 2. Make and attend an appointment at which you will receive vasectomy counseling, undergo financial screening, and sign a permit (consent) for vasectomy. If you qualify, you will be given a referral, and copies of your consent and authorization will be mailed to Dr. Stein,and Galante's office. (If you do not qualify for financial assistance, the standard fee is $690, or $490 if you would like to be part of the Vasectomy Training process.)
3. Call the VasWeb office at 813-536-1430 one week later to see if your paperwork has been received. The VasWeb staff will ask you to watch a Counseling Video, read Vasectomy Information specific to the VasWeb practice, and submit an Online Registration.
4. After submitting the online Registration, call back to VasWeb 813-536-1430 to schedule your vasectomy at their location in Lutz or Kissimmee.on a date 30 days after you signed your consent (that 30-day delay is a requirement for federal assistance).
5. Twelve (12) weeks after your vasectomy, you will mail (mailers provided) a semen sample to the Stein/Galante office in Lutz to confirm that the semen is sperm-free and that you may stop other forms of contraception.
