Port Orange Pediatrics is the office of Ruben Lopez, M.D., who is kind enough to allow VasWeb Doctor Mary Samplaski to use his office periodically (on certain Friday afternoons) to provide vasectomy services. For exact dates, see our Outfield Schedule. The fee is $690, less for men with most health insurance plans, even if they have not met their deductibles. A deposit of $150 is required to be scheduled (see below). Only one visit is required. The counseling process can be done by (1) reviewing our vasectomy page, and (2) viewing the online counseling video. Decide on a date and location from our Schedule, gather your health insurance information, and fill out our Online Registration.

After filling out the Online Registration, call our office at 813-536-1430 to arrange an appointment date and time. Staff is available to take your call between 9 AM and 4 PM, M-F.
Fees
The fee is $690, or, if you have health insurance through a company with whom we have a contract (Blue Cross, Aetna, Cigna, and United HealthCare), the fee is the contacted rate for that company. A deposit of $150, payable during the online registration with VISA/MasterCard is required to be scheduled, unless your vasectomy is covered under Title 10 through a county health department (see Financial Aid below), in which case a deposit is not required. The deposit is fully refundable until 7 calendar days prior to the appointed vasectomy date, non-refundable thereafter for either cancellation or postponement. The balance of the fee ($690, or your insurance company allowabe rate, less $150) is due on the day of the vasectomy, payable by cash check, or credit card.
